MiniSignIn - Collect Attendee Information - iPhone Edition
MiniSignIn lets you configure a simple sign-in sheet for your iPhone. The original app (Sign-In!) is available for the iPad and offers more fields and, of course, larger text. If you have an iPad, youll probably prefer the iPad version. If not, hopefully the 4 fields of MiniSignIn will do what you need.
